Tribunal Nullity Petitions and Other Forms

There are two petitions for submitting cases of marital nullity to the Diocesan Tribunal of Gaylord available on this page.

The first is the Lack of Form Petition. This petition is used to request nullification of marriages involving at least one Catholic party but entered outside canonical form (i.e., not in a Catholic ceremony and not dispensed, subsequently validated, “blessed”, or sanated). This form can be filled out by computer. Do not complete this form in your web browser.  First, download and save the form on your computer. Then use Acrobat Reader to add the required information. If you do not have access to a computer, simply print the forms and provide the information by typing or neatly hand-printing it.  Then return this form and supporting documents through your case sponsor.

Lack of Form Petition

The second is the Petition for a Declaration of Nullity.  This petition is used to request nullification of all other marriages.  This form—as well as the Libellus Form which is to be submitted with it—can be filled out by computer. Do not complete these forms in your web browser.  First, download and save them on your computer. Then use Acrobat Reader to add the required information. If you do not have access to a computer, simply print the forms and provide the information by typing or neatly hand-printing it.

In all cases, the person petitioning for nullity MUST meet with a case sponsor (i.e., a priest, deacon, pastoral administrator, or advocate authorized by the Diocese of Gaylord) prior to submitting a petition for nullity.

Without exception, all petitions must be signed by the petitioner and the sponsor and then mailed with supporting documents to the Tribunal by the case sponsor.

Certificates of Catholic baptisms (if applicable) MUST be issued within six months prior to the date of the petition. 

You are encouraged to keep copies of documents submitted to the Tribunal just in case something gets lost in the mail.
